Qualified Social Qorker

A unique opportunity has arisen for a Qualified Social Worker to join Adults Team in the South West London.

Responsibilities:
1. Carries a caseload of service users referred from Hospital for transfer to one of two pilot facilities to enable suitable patients to move out of acute beds.
2. Undertakes assessments of need and constructs care plans within agreed Departmental procedures and standards.
3. Purchases services to meet assessed needs, seeking the appropriate authorisation from the budget holder. To liaise with providers as required
4. Maintains a knowledge and awareness of legislation and departmental policies and procedures, and to work within these.
5. Works co-operatively with other professionals in the department, in particular with the multi-disciplinary team working with patients in the pilot facilities.
6. Ensures that the service user’s and the carer’s voice is heard throughout the rehabilitation process.
7. Keeps both electronic and manual file records up to date using the department’s recording guidelines.
8. Completes reports as required by departmental procedures.
9. Works in partnership with services users and carers and recommends care plan outcomes based on the service user at the centre of the process, with the least restrictive setting in mind.
10. Attends and participates in meetings regarding service users, e.g. case conferences, network meetings, and reviews.

Pay rate is between £20 and £24 per hour.

GSCC Registration is essential.
